When replacing any part or component in an aircraft, do not assume that the part previously installed is the correct one. Always refer to the manual to verify the correct part number. BA Flt 5390 windshield was replaced before departure, the bolts attaching the w/shield were replaced but with thinner bolts because the AME tried to match with the old bolts which were also the wrong ones. Result: at FL170 W/Shield burst open due to internal cabin pressurization and sucked out the pilot.
